If you fill out our contact or appointment-request form, we collect the information you choose to provide, which may include your name, email address, phone number, and anything you write in the message field. Please do not include sensitive medical details in the website form. The form is for initial contact only.
Information collected automatically. Like most websites, our hosting and security providers may automatically log limited technical information when you visit, such as your IP address, browser type, device type, pages viewed, and the dates and times of your visits, for security and to keep the site running.
We also use analytics to understand how visitors find and use the informational pages of our site, so we can improve them. We use Google Analytics, loaded through Google Tag Manager, and only after you accept analytics through the banner shown when you first visit. We have turned off Google's advertising and cross-device tracking features, Google Analytics does not retain IP addresses, and we do not use this information to build advertising audiences. We do not use any analytics on our appointment or contact forms, and analytics tools never receive information you enter into those forms.

It does not receive any health information, and it does not receive information you submit through our forms.
- Legal and safety reasons, when required by law or to protect the rights, safety, or property of our patients, the public, or the practice.
If any information you submit through the website becomes part of your clinical record, it is then protected as health information under our Notice of Privacy Practices.
How form submissions are handled
Our website contact form is intended for initial, non-urgent contact. Submissions are received within Google Workspace, Google Sheets and Gmail, which we use under a signed Business Associate Agreement. Please do not use the form for emergencies. If you are in crisis, call or text 988, call 911, or go to your nearest emergency room.
Data security
We use reasonable administrative, technical, and physical safeguards designed to protect information collected through the site. No method of transmission over the internet is completely secure, however, and we cannot guarantee absolute security.
Your choices
- You can use the website without submitting any personal information.
- You can choose not to provide information in the contact form, though we then may be unable to respond.
- You can accept or decline analytics through the banner shown when you first visit the site, and you can change your choice at any time. If you decline, the site works exactly the same. You can also control cookies through your browser settings.
